The refactor in 0380656d renamed src/core -> src/core-skills and
src/bmm -> src/bmm-skills but did not update the CLI installer code.
This broke `npx bmad-method install` with ENOENT errors scanning
the old directory paths. Also restores the deleted
src/utility/agent-components/ directory whose fragment templates
are still required by activation-builder.js during agent compilation.

* refactor: consolidate agents into phase-based skill directories
Remove separate agent/workflow/skill directories (src/bmm/agents,
src/bmm/workflows, src/core/skills, src/utility/agent-components) and
reorganize all content into phase-based structures under src/bmm-skills
(1-analysis, 2-plan-workflows, 3-solutioning, 4-implementation) and
src/core-skills. Eliminates the agent/skill distinction by treating
agents as skills within their workflow phase.

* docs: update all references for unified bmad-quick-dev workflow
Remove all references to the old separate bmad-quick-spec and
bmad-quick-dev-new-preview workflows. The new bmad-quick-dev is a
unified workflow that handles intent clarification, planning,
implementation, review, and presentation in a single run.
Updated files across English docs, Chinese translations, source
skill manifests, website diagram, and build tooling.
Replace ambiguous "execute" terminology with explicit "Read fully and follow:"
phrasing across all workflow files to prevent LLM goal-seeking behavior where
models attempt to "achieve the end result" rather than following step-by-step
instructions verbatim.
Changes:
- Update 5 handler templates with canonical phrasing
- Replace ~150 INSTRUCTIONAL patterns across 87 workflow files
- Add "[Workflow] complete." prefix to 7 workflow endpoints
- Preserve BEHAVIORAL/STRUCTURAL patterns (agent descriptions, XML tags)
- Fix gitignore and markdownlint to ignore all node_modules directories
