* refactor: convert advanced-elicitation from XML to markdown workflow
The installer only discovers workflow.yaml and workflow.md files,
so workflow.xml was never registered. Convert to workflow.md with
YAML frontmatter while preserving all elicitation logic verbatim.

file is workflow.md. This broken reference has been present since

Remove hardcoded .bmad folder references throughout documentation and source files, replacing them with the configurable {bmad_folder} placeholder. - Update all documentation to reference {bmad_folder} instead of .bmad
- Remove legacy configuration files from .bmad and .claude directories
