refactor: rename skill folders to bmad-os- prefix

Standardizes all skill folder names with bmad-os- prefix:
- changelog-social → bmad-os-changelog-social
- draft-changelog → bmad-os-draft-changelog
- gh-triage → bmad-os-gh-triage
- release-module → bmad-os-release-module

...
