fix: update all references from workflow.xml to workflow.md
Update 67 references across 50 files to point to the new markdown workflow. Also fix incorrect methods.csv filename and bare advanced-elicitation.xml reference in generate-project-context. Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
