feat: implement GSDMAD super-dev-pipeline v2.0

Implements multi-agent architecture with independent validation:

**New Features:**
- 4-agent pipeline: Builder → Inspector → Reviewer → Fixer
- Independent validation (Inspector verifies Builder's work)
- Adversarial code review (Reviewer wants to find issues)
- Fresh context each phase (no degradation)
- Agent tracking and resume capability
- Final verification by main orchestrator

## Why v2.0?
### The Problem with v1.x
Single agent does ALL steps:
